Terence Tang
Primary Investigator
Dr. Terence Tang is an Internal Medicine physician and a clinician scientist at Trillium Health Partners who focuses on using technology and data to improve care of complex patients, including at care transitions. He has led the development and evaluation of communication technology in the hospital setting, and is looking forward to supporting integrated care across settings involving primary care and the community. He is a lecturer at the University of Toronto, Department of Medicine. He has also been involved in data-driven quality improvement efforts.
