Dr. Carolyn Steele Gray, PhD
Dr. Carolyn Steele Gray's program of work focuses on the role of digital health in supporting integrated and person-centred care delivery for patients with complex care needs. She has studied and published in this topic across the technology life-cycle, including development, implementation and evaluation of digital health solutions. She also co-leads an international Special Interest Group in Digital Health Enabling Integrated Care under the International Federation of Integrated Care (IFIC).

Dr. Terence Tang, MD, FRCPC
Dr. Terence Tang is an Internal Medicine physician and a clinician scientist at Trillium Health Partners who focuses on using technology and data to improve care of complex patients, including at care transitions. He has led the development and evaluation of communication technology in the hospital setting, and is looking forward to supporting integrated care across settings involving primary care and the community. He has also been involved in data-driven quality improvement efforts.

Dr. Michelle Nelson, PhD
Dr. Michelle L. A. Nelson is a Scientist in the Bridgepoint Collaboratory for Research and Innovation in the Lunenfeld-Tanenbaum Research Institute (LTRI) at Sinai Health System and an Assistant Professor in the Institute of Health Policy, Management and Evaluation at the University of Toronto. Michelle leads international research teams focused on rehabilitation and community reintegration for people with complex care needs. She is particularly interested in the role and contributions of volunteers and the voluntary sector in integrated care, transitions from hospital to home, and intersectoral collaboration. Since 2012, she has secured over $5.8 million CDN in research funding as a research lead, principal or co-principal investigator. Her work focused on ‘Voluncaring’ has garnered international attention. She chairs several international working groups and committees in the area, and is a sought after speaker. As a result of this work, she was shortlisted for the 2020/2021 Harkness Fellowship, and has been invited to join the Canadian Foundation for Healthcare Improvement Policy Circle. In 2020, Michelle was appointed the inaugural Chief Knowledge and Innovation Officer for March of Dimes Canada (MODC). Having leadership roles in both research and practice environments creates opportunities for practice informed research, and leverages these insights to advance the design and implementation of evidence informed transitions and community reintegration services.

Hardeep Singh is a Post-doctoral Fellow at the Bridgepoint Collaboratory for Research & Innovation and Trillium Health Partners. Hardeep’s research focuses on developing digital health enabled transition models that are aligned to the priorities and needs of patients, caregivers and health care providers and supporting modernization of care transitions for seniors.

Research Coordinator
Ingryd is a Research Coordinator in the Bridgepoint Collaboratory for Research and Innovation in the Lunenfeld-Tanenbaum Research Institute at Sinai Health. She recently graduated from Applied Clinical Research at McMaster University. She received her Ph.D. in Nursing in 2014 and is a project management professional. She has expertise in quantitative and qualitative research as well as clinical trials. Her previous experiences include coordinating international and multisite research projects, and working with patients, physicians and research staff within primary and complex care settings. At Bridgepoint, Ingryd is supporting multiple projects which focus on implementation and evaluation of digital health solutions, integrated care transitions, and rehabilitation for people with complex care needs.
